This report discusses the June 14, 2003, accident in which the U.S. small passenger vessel Taki- Tooo capsized while attempting to cross the bar at Tillamook Bay, Oregon. A master, deckhand, and 17 passengers were on board the charter fishing vessel when it was struck broadside by a wave and overturned. The master and 10 passengers died in the capsizing; the deckhand and 7 passengers sustained minor injuries. The Taki-Tooo, with a replacement value of $180, 000, was a total loss
